Key Responsibilities Provide practical and emotional support tailoredto each individual’s needs. Assist with daily living activities, including personal care, meal preparation, and medication administration. Support clients in accessing community services, activities, and appointments. Foster independence and encourage individuals to make choices and take part indecision-making. Maintain accurate records of care provided and report any changes in the individual’s needs to the relevant team members or professionals. Collaborate with healthcare professionals, families, and other stakeholders to provide holistic care. Promote a safe and inclusive environmentfor clients and team members. Skills and Qualifications Essential: Genuine passion for helping others and improving their quality of life. Excellent communication and interpersonal skills.
