Department: Children's Services Location: Blackburn& Darwin Working Hours: 5 days per week, 60% office and 40% working from home Requirements: Full UK driving licence and own vehicle Purpose of the Job: Deliver outstanding services to care leavers with energy and enthusiasm. Provide support and interventions to young people aged 16-25, both in and leaving care. Develop positive relationships and offer practical and emotional support based on assessments and pathway plans. Main Areas of Responsibility: Build relationships and communicate effectively to help young people reach their full potential. Support independent living, accommodation options, education, training, employment, health, and social networks. Participate in service development and ensure compliance with legislation and guidance. Collaborate with other agencies to engage young people in opportunities and support. Promote good practice models and adhere to the leaving care offer. Ensure high standards of case recording and administrative records. Promote and develop the Borough’s priorities and participate in training. Knowledge, Skills& Abilities: NVQ level 4 in child care/social care or equivalent qualification. Understanding of leaving care and children legislation, child protection, and safeguarding. Experience working with young people up to 25 years, including those vulnerable to exploitation. Ability to communicate effectively, support resilience, work flexibly, and maintain confidentiality. Commitment to equal opportunities, user involvement, and continuous professional development. Other Requirements: Legally entitled to work in the UK. Willingness to work outside normal office hours. Enhanced DBS check required.
