Our work is solution focused, ensuring that those who most need support receive it and that we can continue supporting people in the future We currently have a vacancy for a grade 9 Specialist Transition Social worker experienced to support individuals with a Learning Disability, Autism and associated mental health diagnosis for young people transitioning from children’s services into adult social care services. You will join an established and experienced team to support the delivery of ASC services to provide consistent and intensive case management during initial stages of a service being set up, and where the professional view is that support can be reduced over a period of time e.g. over the first years of care. It provides a level of assurance for the management team (DMT) in terms of having oversight across packages of support which may involve a deprivation of liberty and where there is a requirement to regularly review any restrictions which may be in place to keep the person safe. You will be based in the High Needs team, at the Guildhall in Hull, working collaboratively with Children’s services, mental health and learning disability health colleagues, to plan, prepare and deliver bespoke services for younger people with complex needs. Through case management, you will be the specialist social worker to inform multi agencies, which will include working in collaboration with children’s services through the transition period from the age of 16 for complex young people.  You will attend Looked after children (LAC) reviews, Education, health and care plan (EHCP) reviews, to celebrate the young person’s achievement and progress through their later years in children’s services, in preparation for their ongoing support and transition into adult social care. You will ensure through ongoing case management that these services continue to meet needs and are best value, proportionate and least restrictive for their assessed need. We are looking for individuals who can demonstrate:  Strong value base that informs your work with adults Relentless commitment to quality - and to be able to talk about what good practice 'looks like' Capacity for honest reflection and openness to learning Ability to establish professional, effective working relationships with a range of partners/colleagues, in children’s services through transitions,  adults, families and their carers. Work as part of a  team and have the ability to work autonomously and have the ability to make complex decisions for individuals who are vulnerable. You will hold a professionalsocial work qualification registered to Social Work Englandand will have experience of assessment and planning, initiating, monitoring, developing and providing outcomes-driven services in adult social care.
