We have an opportunity for an experienced social to work within the rough sleeping and making every adult matter team.
Changing Futures have incorporated a model with a single point of contact, in which a holistic, strengths-based support plan is created for each individual. This allows those living with multiple unmet needs to be able to have input and support from key partner agencies and services through one trusted worker.
Working within the Social Work aspect of this team, you will be responsible for assessing individuals adult social care needs and implementing support as appropriate.
Changing futures Social Workers provide high quality social work practice and manage a caseload with a focus on supporting people facing multiple disadvantages presenting with complex problems including homelessness, substance misuse, contact with the criminal justice system and mental ill health.
Service Vision and Purpose.
Community See& Solve are the main front door integrated service for people accessing Adult Social Care via the community. Within Hull Adult Social Care, we are developing an understanding of trauma informed practice. Trauma-Informed Practice is a strengths-based approach, which seeks to understand and respond to the impact of trauma on people’s lives. The approach emphasises physical, psychological, and emotional safety for everyone and aims to empower individuals to re-establish control of their lives.
Within Adult Social Care there are several initiatives which require working in delivering a coordinated service for people experiencing multiple disadvantages. This includes the Making Every Adult Matter and Rough Sleeping Initiative. The people who are supported within Early Help Adult Social Care Services will require consistent and person-centred social work support to establish care and support needs and reach their long-term personal goals.
Role and Responsibilities of the job
The role will work directly with the housing teams, rough sleepers’ team, mental health homelessness team and other adult social care services to plan, prepare and deliver bespoke services for people with complex needs. To ensure through ongoing case management that these services continue to meet needs and are best value.
The Specialist Social Worker will work as a key part of a multi-disciplinary intervention, the post holder assists people identified within the community see and solve adult social care as experiencing homelessness to improve and maintain their health, by identifying and managing unmet health, housing and social care needs by:
Provide expert and effective social work practice in complex situations ensuring that social work is planned, progressed, and reviewed in accordance with legislative and departmental requirements. That safeguarding process is implemented, and positive risk management approach is core to case management practice.
Navigate and secure services that enable people to remain in control, have access to good information, advice and care.
Leads on local / national strategies for Making Every Adult Matter and Rough Sleepers Initiatives, including workforce development, guidance, policies and procedures ensuring ASC Senior Management Team are kept updated.
Through directly managing a caseload along with management and supervision of staff provide assistance, guidance and decision making regarding managing complex needs and high levels of risk, providing expert opinion regarding law and legal processes e.g. the Court of Protection.
Provides expert leadership in strengths-based approaches to social care, solution focussed conversations, active listening, techniques to agree goals, as well as maintaining high standards of professional practice to ensure the local authorities legislative responsibilities and duties are fulfilled.
The post holder will be able to explain to others about good practice in response to primary legislation and be able to personally demonstrate that good practice. This pertains to legislation such as Mental Capacity Act 2005, Human Rights Act 1998, Care Act 2014 and Mental Health Act 1983.
The successful candidate for Specialist Social Workers will have the following:
Professional qualification in Social Work (Diploma in Social Work, BA/BSc/MA in Social Work, CQSW as required by Social Work England) and is registered with Social Work England.
Experience of working as a social worker with people who have complex needs and require specialist support and interventions to enable them to live safe and wherever possible independent lives.
Experience of assessment and planning, initiating, monitoring, developing and providing outcomes-driven services in adult social care.
A passion to work with people to enable them to live their life and not receive a service
