Develop Care Plans: Create individualized care plans that address clients' specific needs and goals, and regularly review and adjust these plans as necessary. Provide Support and Counseling: Offer emotional support, counseling, and guidance to clients and their families to help them cope with challenges and improve their well-being. Coordinate Services: Work collaboratively with healthcare providers, community organizations, and other professionals to coordinate services and resources for clients. Advocate for Clients: Advocate on behalf of clients to ensure they receive the necessary services and support, including housing, healthcare, and financial assistance. Maintain Records: Keep accurate and up-to-date records of client interactions, assessments, and progress. Crisis Intervention: Provide immediate support and intervention in crisis situations to ensure clients' safety and well-being.
