Patient Care Liaison (classified As a Human Services Assistant)
Annual Pay Range: $52,203.83 - $65,833.47
Hourly Pay Range: $25.097997 - $31.650705
Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

Clackamas County’s Health Centers Division is seeking a dedicated and experienced Patient Care Liaison to support care coordination, patient outreach, and health services administration. This role is essential in ensuring that community members receive the care and resources they need, improving overall health outcomes. If you are passionate about public health, have strong administrative skills, and enjoy working in a collaborative healthcare setting, we encourage you to apply.
Under the supervision of the Population Health Manager, the Patient Care Liaison provides paraprofessional and administrative support for care coordination, case management, and patient advocacy. Responsibilities include conducting client outreach, reviewing eligibility, maintaining patient records, and assisting in health education. This position plays a critical role in supporting population health goals and engaging with stakeholders to enhance service quality.
The most competitive candidates will have experience in healthcare administration, patient advocacy, and care coordination. Strong organizational skills, attention to detail, and familiarity with Electronic Health Records (EHR) are highly desirable. A background in community health, social work, or public health is a plus. The ideal candidate will also have excellent communication skills and the ability to collaborate effectively with patients, providers, and community partners.

Required Minimum Qualifications/ Transferrable Skills:*
• Minimum of two (2) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position
• Experience working in a clinical primary care/healthcare setting
• Experience with Microsoft Office Suite applications and virtual platforms such as Zoom and Microsoft Teams
• Experience using electronic medical records (EMR), such as EPIC or similar systems
• Experience maintaining client and/or program records, including confidential and personal information
• Experience, knowledge and commitment to serving a diverse population and promoting equity, diversity and inclusion
Preferred Special Qualifications/ Transferrable Skills:*
• Experience working within the public sector and/or a government agency
• Experience conducting patient outreach and engaging individuals in healthcare services
• Experience maintaining composure in high-stress situations while working with vulnerable populations
• Bilingual English/Spanish proficiency

TYPICAL TASKS
Duties may include but are not limited to the following:
• Performs outreach: Direct contact, via phone or in writing, with patients and clients to engage them in services, inform them on health education subjects, motivate them to participate in screenings or healthcare activities, or follow up on past care. Track and monitor completion of outreach and outcomes. Coordinate services and multiple types of visits with medical, dental, and behavioral health services.
• Reviews records for eligibility using defined criteria, Pre-Visit screening. Review patient history prior to scheduled Care visit. Adjust Electronic Health Record in coordination with Primary Care Provider, and Care Team. Ensure patient Health Maintenance listings are complete and up-to-date as outlined within Health Centers policy and processes. Audit completion of needed Health Maintenance interventions, and report to Clinical Leadership.
• Care Transition Coordination and follow-up: Follow up after ER/hospital visit. Ensure the patient’s charts are up to date with information on specialist consultations, hospitalizations, ER visits and community organization related to their health. Research current insurance for assignment. Establish care for newly assigned patients at the local Health Center. Analyze chart for Quality Completion of HealthCare Metrics.
• Participate in projects that are identified to improve quality of care, or efficiency of Health Centers services.
• Liaison with stakeholders. Coordinate with internal and external stakeholders in Clackamas Health Centers care coordination activities.
Required Knowledge And Skills
Working knowledge of: Principles and techniques in specific area or field relevant to assigned program; human service agency services associated with assigned area; advanced record keeping procedures and techniques; regulations and ethics associated with program responsibility and confidentiality; techniques of supervision; problems and issues of disadvantaged persons or victims; principles of cultural sensitivity; community resources for clients or victims; basic math; English grammar and composition; principles and techniques of public relations and interpersonal communications; data collection and analysis procedures and methodology; program evaluation; office equipment including personal computers and software programs.
Skill to: Work independently; effectively organize work; lead the work of others; follow written and verbal directions; communicate effectively, both orally and in writing with staff, clients or victims and their support systems, and the public; relate to a wide variety of people of varying ages, socioeconomic backgrounds and needs; apply concepts of cultural sensitivity; maintain records; exercise independent judgment and make decisions within established policies and procedures; develop and conduct trainings and presentations; prepare and present effective written and verbal reports; operate computer software and other office equipment; utilize own ideas to maximize program resources; develop and maintain effective working relationships within the community, law enforcement personnel, with other staff and clients; perform multiple tasks simultaneously.
WORKING CONDITIONS
Some positions may involve exposure to bodily fluids and routine contact with individuals who have communicable diseases. Some positions may involve frequent exposure to hostile and/or disturbed clients.
WORK SCHEDULE
This position is included in the County’s alternate workweek program, working 40 hours 4 days a week (four 10-hour days).
Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department’s business needs. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
